Modalities: Axiatonal Alignment/Re-Alignment, BodyTalk, Reiki, Reflexology, Magnified Healing, Chakra Balance, Complete Balance, Esoteric Healing, IET, EFT, Past Life Regression, Past life/Karma reading, Shamanic Healing, Group Emotional Clearing & Empowerment, Group Axiatonals, Group Reiki Healing Crystal Grid, Group Blessing Bowl Process.
 
I understand that the services provided aim to promote relaxation, well-being, and healing.  I acknowledge that there are no guarantees whatsoever that, through these sessions, I will be healed of any condition I may have at this time.  Holistic Practitioners and Energy Healers do not diagnose conditions, nor do they prescribe substances, perform medical treatments, nor interfere with the treatment of a licensed medical professional.  It is recommended that I see a licensed physician, or licensed health care professional for any physical or psychological ailment I may have. If I am on any prescription medication, I will continue taking it unless my licensed health care provider recommends that I stop.
 
During or after the services provided, the following may be experienced:
Deep relaxation and well-being; sleep or improved sleep patterns; feeling rejuvenated and re-energized, or feeling more fatigued; the temporary experiencing of emotions and/or physical aches, even some that appeared long gone (re. physical pain from past injuries); the disappearance of symptoms that were present prior to the session; personal insights about one’s life, and memories of past events re-surfacing; a deeper sense of spiritual connection, and/or spiritual experiences that are out of the ordinary.  

I understand that everyone is unique, and I may experience other states not listed here during or after the session.  I understand that Heart Journey Wellness & Consulting and the practitioners at the facility are taking all the necessary precautions to maintain clients safe during the pandemic and are following all the city and provincial guidelines. There is no guarantee that this will prevent me from contracting covid19. I agree to wear a mask at all times during in-person sessions until the requirements change in the city and/or province. I take full responsibility for what I may experience during or after the session, and I release and forever discharge the practitioners, Daniele Hart, and Heart Journey Wellness & Consulting, its employees, officers, directors, heirs and successors, from all claims caused by, arising out of, or in any way connected with my participation in this and future sessions with the practitioners at Heart Journey.
 
I certify that I have read and understood the above statements and information. I intend this consent form to cover the entire course of services received from the practitioners of Heart Journey Wellness & Consulting whether in person or through distant healing. By signing this form, I hereby certify that I acknowledge the terms voluntarily and without any duress.
